Before you jump

Sign your
waiver

Everyone entering the park needs a signed waiver, whether they are jumping or watching. It covers all your future visits, and one parent or guardian can add as many of their own children as they need on this single form.

Check this before you go live

This agreement is adapted from the wording already in use at The Arcadium. Have your lawyer confirm it still reads correctly for this location and this activity mix before you accept real signatures — and remember submissions are emailed, not stored in a database. See the notes in src/pages/waiver.html.

1 · Guardian information

I, being 18 years of age or older, am the parent, legal guardian or appointed representative of the child(ren) listed below.

2 · Emergency contact

3 · Children under 18

One adult per waiver — please add minors here. Add every child you are signing for; there is no limit. You must be their parent, legal guardian or appointed representative; you cannot sign for someone else's child.

4 · The agreement

Assumption of risk

I understand that trampolining, climbing, sliding, and the use of foam pits, airbags, obstacle courses and drop towers are inherently dangerous activities that carry a real risk of injury. Those risks include, without limitation: cuts and bruises, sprained or broken wrists and ankles, concussions, dislocations, head and neck injuries, and serious injury to limbs — including serious injuries to limbs that might occur to children on slides. I understand these risks cannot be eliminated by supervision, padding, equipment, court monitors, or any safety measure Arcadium Jump & Climb takes.

I also understand that where people gather there is a risk of transmissible illness, and that during cold and flu seasons there is a raised possibility of catching a cold, flu or other disease.

Release of liability

To the fullest extent permitted by law, I release and agree not to sue Arcadium Jump & Climb, together with its agents, owners, members, shareholders, officers, directors, managers, partners, employees, volunteers, manufacturers, affiliated entities and insurers (the Released Parties), from any claim for loss, damage, injury or death arising from my participation or the participation of any minor named on this form. I agree to indemnify the Released Parties against any claim brought by or on behalf of me or any such minor.

Fitness and sobriety

I confirm I am physically fit to participate and know of no medical condition that makes participation unsafe. I confirm that I have not consumed any alcohol or drugs, or participated in any other activities which might impair my ability to participate safely. I will not participate while pregnant or while injured.

Medical treatment and costs

I authorise the Released Parties to arrange emergency medical treatment for me or any minor named here if it is needed, and I accept responsibility for the cost of that treatment.

Personal property

The Released Parties are not responsible for any lost or stolen items of personal property anywhere on the premises, including items left in lockers.

Rules and staff authority

I have read the park rules and agree to follow them, along with all posted signage, the safety briefing, and every instruction given by staff. I understand court monitors may pause, redirect or end any session at any time, and that being removed for ignoring staff does not entitle me to a refund.

Photography and recording

I grant Arcadium Jump & Climb the right to photograph and record activity in the park and to use those images, unless I decline that permission in the consent section below. Declining does not affect admission.

On behalf of minors

I am 18 years of age or older and I am the parent, legal guardian or appointed representative of every minor named on this form. I have the authority to sign for them and I accept every term of this agreement on their behalf as well as my own.

How long this applies

This agreement applies to all future visits by me, and by each minor participant named here until he or she is 18 years old. You do not need to sign it again on a later visit.

Governing law

The substantive laws of Alberta shall apply to this agreement, and any proceeding shall be brought in Alberta only. If any part of this agreement is found unenforceable, the remainder stays in force.

I have read this agreement, I understand that I am giving up substantial legal rights including the right to sue, and I am signing it freely.
